Clinician Forms, Spa Etiquette and Memberships
Clinician Forms should be filled out prior to your appointment. Our clinician forms are mandatory and must be filled out honestly and completely. No guest may receive a service without the appropriate clinician forms being on file. If you are having trouble filling out your forms – please call spa concierge at 248-686-1259 so we can assist or schedule an additional 10 minutes before your service time to have the forms completed.
For Before Arrival- If you have any preferences, let the staff know in advance. You can do this by adding notes when booking your appointment online, or by calling spa concierge at 248-686-1259. Inclement weather and snow conditions may necessitate reservation cancellations. Guests should use their best judgement regarding safety and travel arrangements. Please be aware of weather and road conditions when making reservations and understand the 24-hour cancellation policy. The treatments you select are reserved especially for you. We require 24-hours’ notice to cancel without incurring the 50% cancellation charge. Please contact spa concierge directly at least 24 hours prior to your scheduled treatment time to modify or reschedule your appointment to avoid penalty.
For Appointment Day- Please arrive up to 10 minutes before your appointment time. Please do not bring valuables to the Spa. We are not responsible for lost or stolen articles. Before leaving for the day, please check that you have not left any personal items behind in your locker or other areas in the spa.
Don’t hesitate to communicate your preferences and needs to your therapist. Whether it’s the pressure of a massage, the temperature of the room, the need for a larger or smaller bolster or any specific areas you’d like to focus on or avoid, clear communication ensures you get the most out of your treatment. Remember, the therapists are there to make your experience as enjoyable as possible, but they can’t read your mind. Please know, we do the best we can to control the HVAC system and keep the room temperature comfortable, but that can sometimes be difficult. We have heaters on our beds and blankets if you are cold and if you are hot, we can add cold stones or remove the heat to help you feel comfortable.
For safety – The Avalon Day Spa is not a suitable environment for those under the age of 18. Many of our services are not designed for children, this includes our concentrated professional skincare products that are not formulated in mind for very young skin. In addition, services like sauna are not designed for young children as their bodies are less efficient at regulating heat, making them more susceptible to overheating and dehydration, which can lead to serious complications like heat stroke. Eucalyptus steam rooms are not recommended for children due to their developing respiratory systems, vulnerability to heat-related illnesses like overheating and dehydration as well.
For atmosphere- The Avalon Day Spa has an extremely strict policy on phone use. Please maintain a peaceful atmosphere by turning off your phone or placing it on silent. Those who do not adhere to this will be asked to turn their phone off or on silent. For guests who have to be reminded a second time in their visit, you may be asked to leave depending on severity of situation. Please keep conversations quiet and avoid loud noises. To respect the privacy of others, no photographs, videos or other social media recordings are allowed inside our spa facilities.
Memberships
When receiving services, all Membership Holders are entitled to (1) beverage including Starbucks House Blend Coffee, FIJI Natural Artesian Bottled Water, or wellness tea blends curated from the Boulder Dushanbe Teahouse in Colorado. Available Blends:
7 Chakra- non caffeinated and blended to support and balance intrinsic human energies. Blend of elderberry, nettle leaf, hawthorn leaf and flowers, blackberry leaf, dandelion root, spearmint leaf, and rosehips. Spearmint adds a touch of cooling, bright aromatics, complemented by the tart, earthiness from the elderberries.
Tranquility- non caffeinated and blended to promote calmness and well being. Blend of peppermint, lemon balm, silver lime flowers, passionflower herb, spearmint, chamomile, lavender flowers, and jasmine flowers.
Daily Detox- non caffeinated and blended to support liver function, cleanse the blood, and move lymph through the body. Blend of dandelion leaf, red clover blossom, nettle leaf, cleavers, peppermint, and burdock root.
Sleep Deep- non caffeinated and blended to promote restful and rejuvenating sleep. Blend of valerian root, spearmint leaf, linden leaf and flowers, chamomile, passionflower, skullcap, and hops flowers.
Gorgeous Goddess- non caffeinated and blended for health of hair, skin and nails. Blend of peppermint leaf, chamomile, nettle leaf, dandelion leaf, burdock root, lemongrass, rosemary, rosehips, bamboo, butterfly pea flower, and hibiscus flowers.

Spa Society Membership is the tier with the highest amount of savings and rewards. Membership is low commitment- for a total of 3 months. Members can choose from a tier list of services that are normally $120.00-$145.00 and receive that service for the membership price of $95.00 for that month. The membership is consecutive and months can not be skipped. Membership is billed on the first day of each month.
Each member is eligible to receive a FREE Spa Society Member Birthday Gift the month of their birthday. No purchase necessary. In addition, if the membership is valid for the month of the member’s birthday- they will receive a $15.00 coupon for retail purchases over $30.00.
